Microsoft’s MAI-DxO diagnosis results are published in Science and go to a hospital study
See how this story connectsMicrosoft AI’s Diagnostic Orchestrator (MAI-DxO) and its Sequential Diagnosis Benchmark (SDBench) were published in Science on 22 October 2026. The benchmark turns 304 New England Journal of Medicine case records and emergency-department cases from MIMIC-IV-ED into interactive cases in which the system or a physician starts with limited information and chooses what to ask or order next. MAI-DxO reached 83% diagnostic accuracy on the NEJM cases against 36% for physicians with textbooks and the internet and 22% without them, and 80% on the emergency-department cases against 47%. One configuration tuned for cost reached 81% on the NEJM cases at less than half the cost of the comparison systems, and the results held when the orchestration was rerun on newer models including GPT-5.5. Microsoft is releasing the benchmark code for selected NEJM cases and will run a prospective study with Beth Israel Deaconess Medical Center in Boston in which patients interact with the system directly and physicians’ independent assessments are the comparison.
Why it matters
Hospitals weighing diagnostic AI get peer-reviewed results on ordinary emergency-department cases as well as rare ones, and the first test on patients starts at Beth Israel Deaconess.
